In addition to the DHSHA Registration Forms, Varsity and Junior Varsity Players are required to provide the DHSHA USA Hockey Registrar a current USA Hockey Registration prior to tryouts.
Varsity Registration Forms
**Varsity Players need the DHSHA Release, OHSAA Physical, and USA Hockey Registration.
Junior Varsity Registration Forms
**Junior Varsity Players need the DHSHA Release, USA Hockey Consent To Treat, USA Hockey Waiver Of Liability, and USA Hockey Registration.
