The three forms below are needed for overnight trips. Note they need to be notarized and given to the team trustees. Please contact your trustees as to when they are needed.

Please note that you will have the opportunity to join Association, Coffman V, Coffman JV,  Jerome, Jerome JV, Scioto, or all email lists.
If at anytime you would like to unsubscribe from an email group, you are able to do so:
1. Log into the website and click on uTools found at the top of the site.
2. Select Group Manager and then Leave the Group(s) you do not want to receive email from.
